How the 2-year lookback works

SSA sets your 2026 premiums from the MAGI on your 2024 tax return. If your income fell since then — you retired, lost a spouse, or your investments dropped — you can file Form SSA-44 to request a new initial determination using your current income instead.

What is IRMAA?

IRMAA (Income-Related Monthly Adjustment Amount) is a surcharge added to your Medicare Part B and Part D premiums when your income exceeds set thresholds. It is calculated by the Social Security Administration based on your tax return from two years earlier.

Why does my 2026 premium depend on my 2024 income?

Medicare uses a two-year lookback. Your 2026 premiums are based on the modified adjusted gross income (MAGI) on your 2024 tax return. If your income has dropped since then due to a life-changing event, you can ask SSA to use your newer income instead.

Can I appeal an IRMAA surcharge?

Yes. If you experienced a life-changing event — such as retirement, marriage, divorce, death of a spouse, or a work reduction — file Form SSA-44 (Medicare Income-Related Monthly Adjustment Amount — Life-Changing Event) with Social Security. SSA will reconsider your premiums using your reduced income.

Does the Part D surcharge replace my plan premium?

No. The Part D IRMAA is added on top of whatever premium your own drug plan charges. You pay the plan premium plus the surcharge, and Medicare bills the surcharge separately.
